We do not accept anonymous complaints.  Complaints without contact information will not be processed.  Complaints without signatures will be returned for a signature whenever possible.

To File a Written Complaint or Request

Click on one of the buttons in the left hand menu. Print the form, sign it and then send it in to us using the information at the top of the form. After signing the form, you may also scan it back into your computer and email it to: mhecker@spokanecity.org
